The following insurance plans are accepted: While I am not a Regence preferred provider, please note that some Regence plans, including Uniform Medical, cover 60% for out-of-network providers. As a courtesy for those with such plans, I can submit the claim to Regence on the patient’s behalf. The amount owed to me by the patient will end up being considerably less than one might expect – about $5 for a return visit, for example. New patients are strongly encouraged to verify insurance benefits and coverage for the treatable condition with their insurance company, as any uncovered expenses are the patient's responsibility.New Patient Intake Form (downloadable PDF)
Cancellation Policy I require three days notice to reschedule or cancel appointments. I charge $75 for late appointment changes or failure to keep an appointment. Please note that I can not bill health insurance for a missed appointment.
Directions From I-5 or crosstown (Greenlake, Phinney): Take the 170 exit (northbound) for 65th/Ravenna. Turn right onto NE Ravenna Blvd, and follow its winding route to the intersection of NE 55th. Turn left on 55th and continue past Metropolitan Market at the corner of 40th Ave. NE. At the first stop sign past Metropolitan Market, turn right at Princeton Ave., following the arterial flow. Turn left onto Sand Point Way, and then almost an immediate right onto 50th Ave. NE. Please note that mapped directions rarely navigate along Ravenna to NE 55th, but it is typically quicker than other routes.

Located off of Sand Point Way in the Laurelhurst neighborhood of NE Seattle. From Sand Point Way, turn onto 50th Ave NE (Chase Bank and a 76 gas station are on the corner). This turn is a half mile north of Seattle Children's Hospital and is near City People's Mercantile. The office is in the yellow triangle building, below the Edward Jones office. The office entrance is on Ivanhoe Place NE, opposite the gas station, down several steps.
