Office hours are by appointment on Tuesday, Thursday, and Friday. The last appointment of the day is at 5:00 pm; the first is at 8 am. The initial appointment fee, if paid at the time of service, is $125. Return visits are $75.

The following insurance plans are accepted:

- Uniform Medical Plan

- Aetna
- PIP (Personal Injury Protection)

New patients are strongly encouraged to verify insurance benefits and coverage for the treatable condition with their insurance company, as any uncovered expenses are the patient's responsibility.

Cancellation Policy
Almost all of my patients are respectful of my time and the time of other patients who want to get in for an appointment. And I understand that emergencies or personal schedule changes can happen that interfere with appointments. But I have developed the following policies to protect the integrity of the appointment schedule.

I require three working days notice to reschedule or cancel appointments. I charge $50.00 for late appointment changes or failure to keep an appointment. Please note that I can not bill health insurance for a missed appointment.
